A clinician decides whether any treatment fits you. ## Your quote checklist for Hormone Logics A usable quote needs three things on every line: an amount, how often you pay it, and what it buys. If any required line is "unknown," your total isn't finished. | Quote line | What Hormone Logics' pages tell us | What's still missing | Exactly what to ask | |---|---|---|---| | First contact | A free consultation is advertised | Who you talk to and what it covers | "Is the free step with a prescribing clinician, and which visit is the first one I pay for?" | | Medical visit and exam | A consult and physical exam are part of the first step | Where it happens and what it costs | "Is the exam in West Palm Beach or by video, and what will I pay for it?" | | First labs | Lab work is required before treatment | Which tests, which lab, and the cost | "Which tests are required, why, and can you use my recent results?" | | Each prescription | No product list for women | Name, route, strength, maker or pharmacy, supply length | "List every medicine and whether each one is FDA-approved or compounded." | | First medicine fill | No posted price | Quantity, days supplied, cost | "What does the first fill cost, and how long should it last?" | | Ongoing care | Monitoring is described | How often, by whom, and the cost | "Which follow-ups and messages are included, and which cost extra?" | | Repeat labs | Not stated | Schedule and price | "Which labs will I need later, and what do they cost?" | | Shipping and supplies | Not stated for women | Delivery fees, applicators, refill fees | "Is shipping included in each fill?" | | Optional extras | The clinic also offers HGH, thyroid, adrenal, supplements and weight-loss services | What's required vs. optional | "Which items are optional, and what is each one for?" | | Billing and commitment | The website terms mention a "membership" but give no billing rules | Due today, renewals, minimum term | "What's charged today, what renews automatically, and is there a minimum?" | | Cancel and transfer | No patient cancellation terms found | Deadline, refunds, records, prescription transfer | "How do I cancel, and what happens to unused medicine, my records and my refills?" | | First 90 days | Can't be calculated from public information | Every required charge | "Show me every required charge in the first 90 days, then the usual ongoing cost." | ## A worksheet you can copy ```text MY HORMONE CLINIC QUOTE (keep this private) Clinic: ______________________ Date asked: __________ Person I spoke with / their role: ______________________ First payment date: __________ First 4-month window ends: __________ (do not include charges on that date) First-year window ends: __________ (do not include charges on that date) Due before treatment: Labs: $______ Charge date: __________ Consult/exam: $______ Charge date: __________ First medicine fill: $______ Charge date: __________ First fill covers: ______ days / calendar months (circle one) Product name(s): ______________________ FDA-approved or compounded? ______________________ Pharmacy that fills it: ______________________ Ongoing — write UNKNOWN when the clinic has not confirmed: Medicine refill: $______ every ______ days / calendar months Next medicine charge date: __________ Follow-up labs: $______ Charge dates: __________________ Follow-up visits: $______ Charge dates: __________________ Membership: $______ Charge dates: __________________ Shipping/supplies: $______ Charge dates: __________________ Other required fees:$______ Charge dates: __________________ Optional extras (keep separate): _____________________________ Minimum commitment / prepaid supply: ___________________________ First 4 months = every required payment dated within that window. Year one = every required payment before the one-year anniversary. Count a prepaid supply in full when charged, even if some lasts longer. If an amount, required status or charge date is UNKNOWN, the total is INCOMPLETE. Don't write $0 unless no charge is confirmed. Cancel rule/deadline: __________ Refund if not prescribed: __________ Refill/records transfer plan: _______________________________________ ``` ## How to count the cost Use the dates you will actually be charged. If the clinic collects three months of medicine at once, count the full payment on that date, even when some medicine lasts beyond your comparison window. Keep the amount due now, the billing interval, the supply length and any minimum commitment separate. Write **UNKNOWN** for a required amount, charge date or fee you have not confirmed. Keep the known subtotal, but mark the whole total **INCOMPLETE**. An unknown cost is not $0. Keep optional extras apart from required care. ## What should you ask Hormone Logics before you pay? Eight questions settle what the reviews can't: the total price before labs, which products you'd get and whether they're FDA-approved, where the visit happens, your state, who prescribes, whether extras like HGH will come up, how your uterus is protected, and how cancelling works. Get the price and product answers in writing. Copy these into your notes for the call. 1. **Total cost.** "What will I pay in total before treatment starts: labs, consult and first medicine?" 2. **The medicine.** "Which exact products would I get, who makes them, and are they FDA-approved or compounded?" 3. **Your pharmacy.** "Can an FDA-approved prescription be sent to my own pharmacy?" 4. **Where.** "Is my first visit in person in West Palm Beach, or by video? Where are my labs drawn?" 5. **Who and where.** "Who is the prescribing clinician, what's their license number, and can they treat me in my state?" 6. **Extras.** "Will HGH, peptides, supplements or weight-loss injections be suggested? Are any required?" 7. **Uterine protection.** "If I have a uterus, what protects my uterine lining, and how is that checked?" 8. **The exit.** "How do refills, follow-up labs and cancelling work, and what does each cost?" **Keep a record:** ```text HORMONE LOGICS — MY ANSWERS (private) Date I asked: __________ Who I spoke with: __________________ 1. Total before treatment: $______ (labs $____ consult $____ medicine $____) 2. Products / FDA-approved or compounded: ________________________ 3. Can I use my own pharmacy? ______ 4. In person or video / where labs are drawn: ____________________ 5. Prescriber name / license / my state OK? ______________________ 6. Extras suggested / required? ___________________________________ 7. How my uterus is protected: ___________________________________ 8. Refills / follow-up labs / cancel rule: ________________________ My decision: book Hormone Logics / compare others / see someone in person ``` Keep your written quote and the clinic's payment, cancellation and refill terms with this record. Requesting a consultation starts a conversation; it does not commit you to treatment. ## Source and scope This worksheet uses the documentation and cost method in the [complete review](/hormone-logics-hrt-reviews-for-women/), checked October 23, 2026. The clinic's [homepage](https://hormonelogics.com/), [bioidentical hormone page](https://hormonelogics.com/bioidentical-hormone-therapy/) and [website terms](https://hormonelogics.com/terms-and-conditions/) inform the quote checklist. Exact prices, products, state access and patient care terms still need a current written answer from the clinic. This sheet is a record of questions and cash charges, not a diagnosis, prescription decision or treatment plan.
