Joi Women's Wellness cost comes in three layers: qualifying lab work, $50 a month for care billed as $150 every three months, and medication on top at $49 to $89 per hormone per month. A new Joi lab panel runs $149 to $699, though Joi says recent outside labs may sometimes qualify instead. An oral estrogen-plus-progesterone plan works out to about $608 for the first 90 days at full retail.

Now here's the part almost nobody tells you.

Two decisions change that total by hundreds of dollars, and you make both of them before you ever speak to a clinician. Which lab tier you buy. And which pharmacy fills your prescription.

There's a third thing. Joi's marketing page says you can cancel any time. Joi's Terms of Use — the contract you agree to at checkout — say you commit to three consecutive months.

We read Joi's product page, lab pages, consultation page, refund policy, and Terms of Use side by side on July 24, 2026. They don't all say the same thing. Below is the real math, every place Joi's own documents disagree, and the exact questions to ask before your card gets charged.

How much does Joi Women's Wellness cost per month?

There is no single monthly price for Joi, and that's the root of the confusion.

Joi's HRT tile shows "$59/mo." That's the starting price of the hormone treatment product, which Joi bills separately from care. The program page lists $50 a month for care and individual medications at $49 to $89 a month. Two different numbers for two different things — neither of which is your all-in cost. None of this is hidden. It's spread across pages that never add up.

The three layers, in order

Layer 1 — Lab work. You pay this first. Joi requires qualifying lab work and clinician review before it will prescribe.

Joi currently advertises 50% off a first lab order for new customers with the codes LABS50 or HRT50. Displayed discounted prices vary slightly between pages. Promotions rotate and can fail at checkout, so every calculation on this page uses full retail — budget from the real number, then enjoy the discount if it applies.

Joi also says recent outside labs may be usable in some cases after review. Worth asking about if you've had bloodwork in the last few months. See the FAQ for what that route currently costs — Joi's own pages disagree on it.

Layer 2 — The care fee. $50 a month, charged as $150 at a time. It covers follow-up labs, clinician visits, and coach check-ins. Follow-up labs are genuinely included — you don't repeat the $149.

Layer 3 — The medication. Billed separately, also quarterly.

These are Joi's published prices, not treatment suggestions. A licensed clinician decides what — if anything — you're prescribed.

What will Joi actually cost in your first 90 days?

Joi publishes prices per month and bills per quarter. That gap is where the surprise lives.

The formula:

The quarterly medication figures are direct multiplication of Joi's posted monthly prices. The patch at $89 × 3 = $267. Progesterone capsules at $54 × 3 = $162. Nothing clever, just the arithmetic Joi doesn't print.

Illustrative first-90-day and first-year totals

All rows use the $149 Complete Panel at full retail. Year one modeled as four $150 care quarters. Excludes promotions, taxes, shipping, at-home draws, and state-specific pricing.

Two decisions that move the number

• Upgrade to the Advanced Panel: add $250.
• Upgrade to the Comprehensive Panel: add $550.
• If a verified 50% promotion applies: subtract the actual displayed discount from your lab charge. Don't build a budget on it.

So the same oral estrogen-plus-progesterone plan runs about $608 on the Complete Panel or $858 if you start on the Advanced Panel. Same treatment. A $250 spread, decided at the very first click — which is exactly why the biomarker section below matters more than it looks.

What does the $149 lab test — and what does it leave out?

Why this is a money problem, not just a testing problem

In its own written response to a Better Business Bureau complaint (November 6, 2025), Joi said a processed bundled panel could not be retroactively modified or reordered, and that responsibility for confirming what's included rests with the patient. That complaint involved a woman who expected a cholesterol panel, was told to fast, and didn't get one.

So if a missing marker turns out to matter, you don't add it to the order you already placed. You place a separate order. The $250 gap between Complete and Advanced isn't a comfort upgrade. It's the price of a decision that's hard to undo.

The honest other side

A bigger hormone panel is not automatically better menopause care. Current FDA labeling for estradiol gel states that serum FSH and estradiol levels are not useful in managing moderate-to-severe hot flashes and night sweats. ACOG takes a similar line: hormone testing generally isn't needed before standard treatment, because symptoms, cycle changes, and history do the work.

What's usually non-refundable — and the exceptions Joi publishes

Joi's refund policy states that prescription products can't be returned and that sales are final. Hormone therapy subscriptions can be cancelled but not refunded. Diagnostic lab sales are final. The Terms add: "All Products are non-refundable."

There are published exceptions. Joi states elsewhere that a refund may apply when a licensed provider determines a prescription is inappropriate, and that mobile phlebotomy charges are refunded when that service can't be delivered. What Joi does not clearly state is which components a provider-declined refund covers.

The honest downside — and why it might not matter to you

Joi does NOT offer flat, predictable monthly billing, and completed labs, paid subscription charges, and fulfilled prescriptions are generally non-refundable. If a fixed monthly-equivalent price is what you need, a different structure may work better for you.

But because Joi separates care from medication instead of bundling them, you can see every line item — and you may be able to swap one out. The single biggest saving available to a Joi patient exists only because the medication is priced apart from the care. A bundled plan can't be unbundled.

Does Joi Women's Wellness take insurance?

Joi's Terms of Use say it plainly: Joi is "not enrolled with or otherwise acting as a participating provider in any federal or state healthcare programs (i.e., Medicare, Medicaid)." So the labs, the care fee, and any compounded medication are cash. The part that isn't simple is worth hundreds of dollars a year.

The local pharmacy route

Joi's published policy — stated on three separate pages — is that women's health medications like estrogen and progesterone can be sent to your local pharmacy, where you may use your insurance benefits. Joi's current landing page puts a price on it: a $35 fee per 3-month prescription.

HSA and FSA

Joi accepts HSA and FSA payment and will provide a detailed receipt plus a Letter of Medical Necessity from your provider on request. Joi is honest about the limit: it makes no warranty that your administrator will accept the charge. Ask whether a Letter of Medical Necessity is required — Joi says Flex may request one, and it doesn't guarantee reimbursement or plan acceptance.

If you have a PPO, check that before you pay cash anywhere

Joi's cash-pay model is a real cost for insured women. If your plan would cover a menopause visit, paying out of pocket for labs is money you may not need to spend.

Are Joi's hormones FDA-approved or compounded?

"Some drugs may include compounded products that have not been approved by the FDA. The FDA does not verify the safety or effectiveness of compounded drugs including compounded Women's Hormone Therapy. Our specialists may prescribe this medication off label from its intended use." — Joi's Important Safety Information

That's a blanket disclosure — it doesn't tell you which specific item in your cart is compounded, and it doesn't establish that everything is.

What the words actually mean

• FDA-approved describes one specific finished medicine: a particular strength, in a particular form, made by a particular manufacturer, with labeling the FDA reviewed.
• Compounded means a licensed pharmacist or qualifying facility prepares or alters a medication for a patient-specific need. The finished compounded drug is not FDA-approved and the FDA does not review it for safety, effectiveness, or manufacturing quality before it reaches you.
• "Bioidentical" is not an FDA approval category. Some FDA-approved estradiol and progesterone products are described as bioidentical, and so are some compounded products. The word tells you nothing about approval status.

Who actually makes your medicine — Joi names them

That list is a sourcing disclosure, not product-status evidence. A pharmacy's name or business model doesn't establish whether the specific item in your cart is compounded — but it tells you exactly who to ask about, which is more than most readers ever get.

The Terms also disclose that certain Joi shareholders, directors, officers, or agents "may have a financial interest in one or more third parties providing the Services or Products, and may profit from your use and/or the sale of such Services and Products." That's useful context when you're weighing an in-house medication price against your local pharmacy's.

Two claims on Joi's site worth asking about

One. Joi's FAQ says the 2002 Women's Health Initiative study "has been proven wrong." That's stronger than where the evidence sits. In February 2026, the FDA removed some statements from the boxed warning for six menopausal hormone therapy products — a genuine, meaningful shift. It supports a more current reading of the WHI evidence. It doesn't make every original WHI finding "proven wrong."

Two, and this one costs money. Joi's FAQ says progesterone is necessary even if you don't have a uterus, citing brain, mood, and sleep benefits. A progestogen is generally added to systemic estrogen to protect the endometrium; after a total hysterectomy it's generally not required for that purpose, though a clinician may have another individualized reason. Joi's position adds $54 to $69 a month — $648 to $828 a year. If you don't have a uterus and progesterone is on your plan, ask your clinician to explain why. It's a fair question.

The one question to ask Joi before you pay

This is the highest-value thing on this page. Three of Joi's documents point one way. One points the other.

A customer review posted around April 2026 matches the BBB version: she paid the lab fee, got results, and says she was refused her chosen pharmacy.

The weight of Joi's own published documents supports the local-pharmacy route. But the marketing page is the one you read before you pay, and at least one customer's experience didn't match it. The current evidence supports requesting that route — not assuming any named pharmacy will be accepted.

Where Joi can't ship testosterone — and the state on one list but not the other

Joi attributes these restrictions to state regulations. Testosterone is a Schedule III controlled substance, and shipping rules vary — this pattern is common across telehealth, not unique to Joi.

One state-specific right most women never hear about

Joi's Terms of Use contain this line:

Patients residing in New York, New Jersey, and Rhode Island have the right under their respective state patient billing laws to request an itemized price list from Joi for their laboratory tests.

If you live in one of those three states, you can ask for itemized lab pricing in writing. Joi also confirms that New York and New Jersey have different pricing for both products and labs, and that draws there go through BioReference rather than Quest. If that's you, request the itemized list — it's the fastest way to get a real number.

How does Joi's cost compare with other HRT care models?

So who wins?

Nobody wins in general. People win in specifics:

• You want bloodwork, a real conversation, and ongoing monitoring bundled in → Joi is genuinely built for that, and few competitors fold follow-up labs into the care fee the way it does.
• You want the lowest predictable monthly-equivalent price with FDA-approved products → Hers.
• You have a PPO → Midi, before you spend a dollar of cash anywhere.
• You want the cheapest cash price and no lab requirement → Winona, confirming the exact product's status rather than assuming.
• You want a low-cost subscription with labs included → Sesame, unless you're on Medicare, Medicaid, or TRICARE, or you're seeking testosterone.

What do Joi customers actually say?

One caveat first: the Trustpilot listing is a company-level profile that has been merged with one or more other profiles. It is not a women's-HRT-only rating. The smaller aggregator sites are running samples in the thirties. Don't average any of it.

What women praise. The recurring theme isn't the medication. It's being taken seriously. One Trustpilot reviewer wrote that her OB-GYN refused to run labs, told her she was "too young for perimenopause," and offered antidepressants instead. Another, on RealReviews, said that after four in-person doctors ignored her, she "finally felt heard."

What women complain about. The same RealReviews writer who felt heard also warned that the visit "goes fast," and advised writing your questions down first. A detailed June 2026 Trustpilot review describes ending up with a 90-day medication supply and a significant financial commitment, now having a new OB-GYN review what was prescribed. And a reviews.io customer from April 2026 is the one who reported being refused her chosen pharmacy.

None of these establish whether hormone therapy will work for you. They're useful for setting expectations about the service — appointment length, response times, and how much homework to do before your visit.

What's still unconfirmed before checkout?

Joi's public pages don't resolve everything. Before you pay, confirm in writing:

• The exact date each charge first hits your card, and the next processing date
• The full scope of a refund if a clinician decides not to prescribe
• Whether the $35 local-pharmacy fee applies separately to each medication, every quarter
• Which catalog products are compounded and which are FDA-approved
• Whether shipping is included in the medication price
• Which named local pharmacies Joi will actually send a prescription to
• Which price applies if you want recent outside labs reviewed instead of buying a new panel

Is Joi Women's Wellness worth the cost?

Joi is not a $50-a-month medication subscription, and treating it like one is how women get surprised. It's a cash-pay program. The modeled full-retail baseline before medication is $749 for year one. The medicated scenarios on this page run from $1,337 to $3,173, and an eligible local-pharmacy route may land lower depending on routing fees and pharmacy pricing.

If you want comprehensive bloodwork, a listed 30-minute clinician consultation focused on your results, and monitoring built into the price — and you can absorb a non-refundable quarter — that's a reasonable thing to buy. Joi is genuinely built to deliver it.
