Menopause and Hearing Loss: What 9 Studies Actually Found — and What to Do About It

Route the hearing concern before changing hormones

Menopause and hearing loss are linked in research, but menopause has not been proven to independently cause hearing loss. Two U.S. analyses found no independent menopausal-status association after adjustment; a U.K. diagnosis study found adjusted associations. Hearing can change across midlife. HRT is not a hearing treatment. A sudden drop — especially in one ear — needs same-day medical care.

That's the honest headline. Now here's the part almost nobody explains: the studies on this question look like they contradict each other, and much of the conflict comes from what they measured. Once you see the split between hearing tests, self-reports, diagnosis codes, and repeated testing over time, the pile of headlines starts to make sense — and so does what you should actually do next.

Get care today, not next week, if any of this is true

A sudden hearing drop is a medical emergency, not a menopause symptom to watch at home. Same-day assessment matters because sudden sensorineural hearing loss can feel like wax or fullness, and treatment is time-sensitive. Neurological symptoms, severe vertigo, or head injury raise the urgency further.

  • New hearing loss plus facial weakness, trouble speaking, a severe headache, or new numbness
  • New hearing loss plus sudden severe vertigo
  • New hearing loss after a head injury

Why the rush? Sudden sensorineural hearing loss is a rapid drop caused by a problem in the inner ear or hearing nerve. A commonly used diagnostic definition is a loss of at least 30 decibels across three neighboring frequencies within 72 hours, but subtler sudden changes can still qualify and still need urgent assessment.

The treatment window was easy to misstate, so here is the current guideline language precisely: hearing should be confirmed with audiometry as soon as possible and within 14 days; corticosteroids may be offered as initial treatment within two weeks; and intratympanic steroid salvage is recommended for incomplete recovery two to six weeks after onset. That does not mean you should wait two weeks. The safest reader action is same-day assessment because delay can reduce treatment effectiveness. (AAO-HNS guideline; NIDCD)

Do not wait to see whether it's menopause.

*What is not automatically an emergency:*

  • Gradual difficulty in both ears, especially in noisy places
  • Long-standing trouble that has not changed
  • A day or two of muffling with an obvious cold or blocked nose
  • Ringing without a sudden hearing change

Those patterns can still need care. New one-sided tinnitus, persistent fullness, fluctuating hearing, pain, drainage, or a clear difference between ears deserves prompt assessment even when the change was not sudden. This is not a complete list of emergency symptoms. When you're unsure, get checked.


Can menopause cause hearing loss?

Menopause has not been established as an independent cause of hearing loss. Two U.S. studies found no significant independent association after adjustment, while a large U.K. diagnosis-code study found adjusted associations. Repeated testing also shows hearing can decline across these years. The evidence is mixed on association and does not prove menopause caused an individual's change.

Here's the thing that makes this question so slippery: menopause and age-related hearing loss occupy the same stretch of the calendar.

The average age of menopause in the United States is 52, and the usual range is 45 to 58. Age-related hearing change also accumulates over time, commonly affecting higher frequencies first. That makes the two look connected before a study has done anything to separate them. (Office on Women's Health)

The research question is whether menopause is doing it, or simply happening nearby.

Two U.S. studies took that question seriously.

The Nurses' Health Study II followed 80,972 American women for 22 years. When researchers compared postmenopausal with premenopausal women and adjusted finely for age and other factors, natural menopause carried a relative risk of 1.00 (95% CI 0.94–1.05). Surgical menopause was 0.95 (0.89–1.02). A relative risk of 1.00 means the groups had the same measured risk in that analysis. (Curhan et al., 2017)

Then in April 2026, a USC-led team published a national analysis using actual hearing tests rather than asking women how their hearing seemed. After adjustment, postmenopausal status was associated with a +0.92 dB difference (95% CI −1.02 to 2.86), which was not statistically significant.

Before adjustment, the raw difference was 5.78 dB. After age and other factors entered the model, it fell to 0.92 dB. That is strong evidence that age and related characteristics explain much of the crude difference in that dataset. It is not proof that hormones play no role in any woman. (Hsu et al., 2026)

What this does not mean: it does not mean you're imagining things. The studies that followed the same women through the menopausal transition did record change. They simply cannot, by themselves, prove that menopause rather than aging caused it.


When is a hearing change an emergency?

A sudden drop over hours to days needs same-day medical care, particularly when one ear is affected. Gradual, roughly even difficulty in noisy rooms is usually not an emergency, but it still deserves a baseline evaluation. The three clues that matter most are speed, sidedness, and whether neurological symptoms came with it.

Speed matters most. Hearing that fades over months behaves differently from hearing that drops over a weekend.

Sidedness matters next. Age-related change is commonly bilateral, although the ears are not always identical. A clear new difference between ears is not something to write off as menopause, and it can point to a specific cause that needs evaluation.

Anything neurological — face drooping, slurred speech, new weakness or numbness, a thunderclap headache, or severe sudden imbalance — is an emergency call, full stop. Do not use any website, including this one, to talk yourself out of that.

One more thing worth saying plainly. If your ear feels blocked and you assume it's wax, allergies, or “just hormones,” you may be right. But an ear exam can only look for conductive causes such as wax or fluid; it cannot rule out sensorineural loss by itself. Air- and bone-conduction audiometry helps separate conductive from sensorineural patterns. (NIDCD)

What are the 7 signs women actually notice around menopause?

Hearing change in midlife rarely starts with silence. It starts with effort. The first complaint is often clarity rather than volume: you can tell someone is speaking, but separating the words becomes hard when noise is behind them. Six other patterns recur, and each one changes what should be tested or how quickly it should be checked.

Here's what women describe, and what each sign can mean.

1. Restaurants have become impossible. This is the classic first sign, and it's the one most often dismissed. Background noise removes the redundancy your brain uses to fill in gaps. Even a small high-frequency change can show up there before it causes obvious trouble in quiet.

2. You can hear that someone's talking, but not what they said. Consonants such as s, f, th, and sh carry much of the detail in speech and sit higher in frequency than many vowels. You get the shape of the sentence and lose the edges.

3. Women's and children's voices are harder than men's. Higher-pitched voices can sit closer to the frequencies where age- and noise-related loss often begins. That pattern is useful information for an audiologist, not a personality trait.

4. You're exhausted after a day of meetings. Listening fatigue is real. When the signal is degraded, the brain has to do more reconstruction. Some people notice the effort before they identify it as hearing difficulty.

5. Someone else has mentioned the TV volume. You may not notice a slow drift. The people around you often do. It's an unglamorous but genuinely useful data point.

6. One ear feels blocked or “full,” and nothing comes out. This is different from ordinary both-sided difficulty. Wax, fluid, pressure problems, inner-ear conditions, and sudden sensorineural loss can all feel “blocked.” One-sided or sudden fullness deserves assessment rather than waiting.

7. You've started avoiding things. Skipping the group dinner. Choosing the quiet table. Emailing instead of calling. This is the sign with the biggest life cost, and it is often the one women mention last.

The FDA's OTC hearing-aid guidance uses much of the same symptom language — trouble in noise, trouble in groups, phone difficulty, listening fatigue, and a TV volume other people find too loud. Its medical red flags include sudden, rapidly worsening, fluctuating, or one-sided loss; vertigo; pain or drainage; and one-sided tinnitus. (FDA OTC hearing-aid guidance)

What do 9 studies on menopause and hearing loss actually show?

Nine selected studies make up the evidence matrix below, and they do not all point in one direction. They used hearing tests, self-reports, diagnosis codes, snapshots, and repeated measurements. Put the methods beside the findings and the conflict becomes useful instead of frightening.

⚠️ One set of numbers needs a warning label, but not erasure. Fu's study reported adjusted hazard ratios of roughly 2.5 for natural menopause and 2.7 for surgical menopause compared with premenopause. Those are real results from its model. They are also diagnosis-code associations in an observational UK Biobank cohort, not proof that menopause nearly triples hearing loss or a personal risk estimate you can apply to yourself.

That result sits beside two U.S. analyses in which menopausal status was not independently associated after adjustment. The outcomes and designs differ enough that one study cannot simply cancel the other. The correct conclusion is that the epidemiology is mixed — not that the largest-looking ratio automatically wins, and not that it can be dismissed as a raw age comparison.

We're telling you this about a study we're citing. That's the point of the limitations column.


Why do the studies disagree?

They disagree partly because they measured different outcomes — and partly because the evidence is genuinely unsettled. Repeated audiometry can show when hearing changes. Self-reports capture noticed difficulty. Diagnosis codes capture who reached clinical care. Cross-sectional hearing tests compare groups at one point. None answers every part of the question.

Look at the matrix again with that lens.

Group one — follow the same women over time: Hederstierna, Svedbrant, and Jang all recorded decline. These designs are good at showing trajectories. They are weaker at separating a menopause effect from aging, cohort effects, and other changes happening over the same years.

Group two — compare menopausal groups while accounting for age and other factors: Curhan and Hsu found that menopausal status itself was not independently associated. Curhan used self-report; Hsu used audiometry. Neither proves that hormone changes are irrelevant to every woman.

Group three — follow diagnoses across reproductive histories: Fu found associations with premature menopause, parity, reproductive lifespan, and menopausal status. That design reaches enormous scale, but a diagnosis code is not the same endpoint as an audiogram.

So here's the honest synthesis:

Your hearing may be changing across the menopausal years. The evidence has not established that menopause itself, rather than aging and connected health factors, is the independent cause.

That sounds like a technicality. It isn't. It changes what you do.

If you believe menopause is the cause, the logical move is to fix the hormones and expect your hearing to follow. That's the reasoning most of the internet nudges you toward, and it is the reasoning most likely to waste time — because the hormone evidence does not support HRT as a hearing treatment.

If you understand that the cause still needs finding, the plan becomes different: measure the hearing, look for reversible and modifiable contributors, and treat menopause symptoms on their own merits.

Same uncertainty. A much better plan.


Does later menopause protect your hearing?

The two large U.S. studies point in opposite directions, and neither gives you a clinical rule. The Nurses' Health Study found slightly higher self-reported risk with later natural menopause. The 2026 audiometry study found better measured thresholds with later menopause and a longer reproductive lifespan. The disagreement is real.

Why would the same variable point two ways?

One plausible explanation is the outcome. Self-report captures whether you noticed and reported difficulty. Audiometry captures thresholds at selected frequencies in a quiet setting. Those are not interchangeable. Women who reach menopause later may also differ in health, exposure, genetics, and care-seeking in ways no model can fully remove.

Curhan's team called its later-menopause finding unexpected and said the mechanism was unclear. Hsu's team said the clinical relevance of modest threshold differences needs further study.

The practical answer is simple: age at menopause does not tell you whether your current hearing problem is hormonal, reversible, or treatable. It belongs in your history, not in a self-treatment formula.

We could have picked whichever study made a cleaner story. We'd rather you saw both.


What does estrogen actually do in the ear?

Estrogen signaling exists in the inner ear, so a hormone–hearing connection is biologically plausible. Human tissue research has found estrogen receptors in cochlear structures, and animal experiments show that changing estrogen signaling can change hearing. The experiments also point in opposite directions, which is exactly why mechanism cannot substitute for human treatment evidence.

In adult human inner-ear tissue, researchers identified estrogen receptor alpha in spiral ganglion neurons and estrogen receptor beta in the stria vascularis. The stria vascularis helps maintain the ionic environment the cochlea needs to turn sound into nerve signals. (Stenberg et al., 2001)

Animal work broadens that map and makes the biology look compelling. Mice lacking estrogen receptor beta developed severe age-related hearing problems and inner-ear pathology. (Simonoska et al., 2009)

Then the biology refuses to stay tidy.

In a mouse model of adult-onset hearing loss, estrogen alone and estrogen-plus-progestin worsened aspects of cochlear degeneration, while removing ovarian hormone exposure reduced some damage. (Price et al., 2009)

That is the counterweight most articles leave out. A receptor proves that a tissue can respond to a hormone. It does not tell you whether adding that hormone helps, harms, or does nothing in a woman with gradual hearing difficulty.

The mechanism is plausible. The treatment conclusion is not established.


Does HRT help hearing loss — or make it worse?

HRT is not a treatment for hearing loss, and no FDA-approved menopausal hormone therapy is indicated for it. Curhan linked longer oral use with higher self-reported risk; Fu found higher diagnosis risk in one MHT subgroup; Hsu found no significant association. None showed hearing benefit or supports changing HRT for hearing alone. (FDA; Curhan 2017; Fu 2025; Hsu 2026)

Our one damaging admission

We'd rather you heard this from us than found it in a study abstract at midnight.

There is no version of the human evidence in which hormone therapy is an established treatment for your hearing. Curhan's large cohort pointed slightly the wrong way for estrogen alone as well as combined therapy, Fu found higher diagnosis risk in one MHT subgroup, and Hsu found no significant association. If protecting your hearing is your reason for considering HRT, that's not a good reason. Close this page and book the hearing test instead.

And here's why that's genuinely good news for you.

Because HRT is not the hearing lever, you do not have to solve the hormone question before you do anything. The useful actions are available now:

  • A diagnostic hearing evaluation
  • A medication review
  • A check of whether bone-health screening is due for your age and risk
  • Blood-pressure and blood-sugar follow-up when clinically indicated
  • Protection from damaging noise
  • Amplification or other rehabilitation if testing shows a need

None of that waits on a hormone decision.

And if you're already taking HRT for hot flashes, sleep disruption, or another menopause indication — this page is not a reason to stop. Symptoms may return after discontinuation, and stopping offers no proven hearing benefit. Bring the hearing concern to your prescriber and evaluate your ears separately.

Does the type or route of HRT change anything?

No route or formulation has been proven to protect hearing. The claim that estrogen helps while progestin harms comes largely from a small 2006 comparison, but the 80,972-woman prospective cohort did not reproduce that pattern. Its transdermal results were described as similar to oral results, although patch use was less common.

These numbers come from Tables 4 and 5 of the full Curhan paper.

The shorter-duration estimates were not clearly significant. That is a meaningful detail the headlines dropped.

Read those first two rows again. Estrogen alone was not lower than combined therapy in this dataset. The neat “progestin is the villain” story does not survive the larger prospective comparison.

And on patches: transdermal use was less common, but the paper states that its associations were similar to oral use. That does not prove patches carry hearing risk. It does mean “switch to a patch for your hearing” is not supported by this study.

Three things this section is not saying

It is not saying stop or switch your HRT. Do not change a prescribed dose or route because of a webpage.

It is not saying remove endometrial protection. For a woman with an intact uterus using systemic estrogen, adequate progestogen is generally recommended to reduce endometrial risk. Recommended low-dose vaginal estrogen for genitourinary symptoms is a separate category and generally does not require a progestogen. (The Menopause Society 2022 position statement)

It is not saying the hearing numbers are settled. They are observational associations with modest effect sizes, self-reported hearing, and a cohort that was more than 94% White. What else could be causing this?

Several causes of midlife hearing change are more findable — and sometimes more fixable — than hormones. Wax, middle-ear problems, noise damage, medication effects, and inner-ear conditions can produce overlapping symptoms. The pattern narrows the possibilities, but an ear exam and hearing test do the separating.

The table is a routing aid, not a self-diagnosis. Otosclerosis, Ménière's disease, and vestibular schwannoma are distinct conditions, and the same word — fullness, muffling, ringing — can appear in more than one of them. (NIDCD hearing conditions)

On medication effects, one caution matters more than any drug list. Do not stop a prescription because you saw its name beside the word ototoxic. If the hearing change followed a new medicine, a dose change, or a course of treatment, bring the exact product, dose, start date, and symptom date to the prescriber or pharmacist. The timeline is the useful evidence.

Notice what the table keeps asking for: an exam, a test, or both. That's the argument for measuring instead of guessing.


What else raises hearing-loss risk in midlife?

Noise exposure, medication history, cardiovascular and metabolic health, and low bone density are more actionable than menopausal status alone. That does not make each one the cause of your hearing change. It means they can be documented, reviewed, and addressed without waiting for the hormone research to settle.

The cardiovascular finding is the one most likely to be overread.

In the UK Biobank study of 214,327 women, a statistical mediation model estimated that cardiovascular disease accounted for 46.07% of the association between four or more childbirths and hearing-loss diagnosis, and 19.61% of the association between premature menopause and hearing-loss diagnosis. (Fu et al., 2025)

That does not prove the signal “travelled through the blood vessels,” and it does not prove that treating cardiovascular disease will prevent hearing loss. Mediation analysis estimates how associations fit together under model assumptions; it is not a treatment experiment.

The Korean repeated-audiometry study points in a compatible but still observational direction: women in the poorer hearing trajectory were more likely to have overweight or obesity and hyperglycemia. (Jang et al., 2025)

Two large cohorts, two different designs, a reason to take general health seriously — not permission to turn association into a cure.

The useful move is not to decide which row “caused” it before the appointment. The useful move is to arrive with enough evidence that the clinician does not have to start from fog.


Is low bone density linked to hearing loss? The bone connection nobody mentions

Low bone density and osteoporosis were associated with up to about a 40% higher risk of moderate-or-worse hearing loss in two cohorts totaling 143,899 women. The study did not prove that bone loss caused hearing loss, and bisphosphonate use did not change the association. It is a real signal with a hard limit.

The analysis combined the Nurses' Health Study — 60,821 women followed from 1982 to 2016 — and Nurses' Health Study II — 83,078 women followed from 1995 to 2017. A history of vertebral fracture was also associated with higher risk; hip fracture was not. The outcome was self-reported hearing loss, not measured thresholds. (Curhan et al., 2021)

This is one of the clearest measurable associations in this evidence set, and it has nothing to do with using HRT as an ear treatment.

Why might bone and hearing travel together? The inner ear is encased in bone, and bone metabolism, vascular health, age, and other systemic factors may overlap. The study cannot tell us which mechanism explains the association.

The bisphosphonate result is the damaging detail inside the interesting finding: women who used those medications did not have a different hearing-loss risk from women who did not. That argues against the easy story that treating bone density automatically protects hearing.

Here is the part that changes an appointment.

Current USPSTF guidance recommends osteoporosis screening for all women 65 and older and for postmenopausal women younger than 65 who are at increased fracture risk after clinical risk assessment. It does not recommend a bone-density scan for every woman simply because she has reached menopause or noticed a hearing change. (USPSTF osteoporosis screening recommendation, 2025)

So do not ask for a bone test “for your ears.” Ask whether you meet bone-screening criteria for your age and risk history. If that part of midlife care has been skipped, close the gap for the reason the test is designed to address — fracture prevention — and treat the hearing question separately.


I can hear the beeps but I still can't follow speech. What's going on?

A pure-tone test can look normal or near-normal while real conversation still feels exhausting. Beeps in a quiet booth measure the softest tones you detect at individual frequencies. They do not fully reproduce competing voices, reverberation, visual distraction, attention, or the work of separating speech from noise.

This is the single most invalidating experience in this whole topic. You do the test, someone says the numbers are “normal,” and you go home still unable to hear your friend across a restaurant table.

The 2026 U.S. study on this page used a better-ear pure-tone average at 0.5, 1, 2, and 4 kHz. That is a useful standard measure. It is also an average of four frequencies, in the better ear, in controlled conditions. (Hsu et al., 2026)

A small high-frequency change can be diluted inside an average. More importantly, understanding speech in noise depends on more than whether you can detect a tone. That is why the complaint and the test result can both be honest.

What to do about it: ask whether speech-in-noise testing is appropriate for your complaint. Ask to see each ear's thresholds across the full frequencies tested, not just the average. Ask for a copy of the audiogram and speech results so future testing has a documented comparison point.

Take this sentence into the appointment:

“My main difficulty is understanding speech in background noise. Can we test that directly, even if my pure-tone thresholds are in the normal range?”

That is more accurate than telling the clinician what your diagnosis must be. It names the failure point and asks for the test that resembles it.


Is hearing loss really linked to dementia?

Yes, hearing loss is treated as a potentially modifiable dementia risk factor — but that is not the same as saying it causes dementia or that a hearing aid guarantees prevention. The 2024 Lancet Commission assigned hearing loss a 7% weighted population-attributable fraction. The major U.S. randomized trial was negative in its overall primary analysis.

Let's do this carefully, because this is the fear that brings a lot of women to this page at 1 a.m.

What the Lancet Commission estimated. Its life-course model included 14 potentially modifiable factors with a combined weighted population-attributable fraction of 45.3%. Hearing loss was classified as a midlife factor with an estimated relative risk of 1.4 (95% CI 1.0–1.9) and a 7% weighted fraction. The authors also said some associations might be only partly causal. A population-attributable fraction is a model of population-level potential under assumptions — not a forecast for one woman. (2024 Lancet Commission)

What the ACHIEVE trial found. The trial randomized 977 adults ages 70 to 84 with untreated hearing loss to a hearing intervention or health-education control and followed them for three years. In the full study population, the intervention did not significantly slow cognitive decline. (Lin et al., 2023)

The 48% result came from a prespecified subgroup: 238 participants already enrolled in a cardiovascular-health cohort who were older on average and had more cognitive-risk factors. In the 739 healthier community volunteers, the intervention did not reduce cognitive decline over three years.

A later secondary analysis regrouped people by predicted cognitive-decline risk and found greater benefit in the highest-risk quartile. That refines who may benefit; it does not rewrite the original trial into an overall positive result. (Pike et al., 2025)

And the detail that matters most for this page: ACHIEVE enrolled adults 70 to 84, not women in their 40s or 50s navigating menopause.

So what is fair to conclude?

Hearing loss is associated with dementia risk strongly enough to justify serious research and public-health attention. Treating hearing loss may help cognition in some older, higher-risk adults, but prevention is not guaranteed and the evidence cannot be copied onto a 52-year-old. Hearing care still has immediate value: clearer communication, less daily effort, better access to the people around you.

Being able to hear your people is enough reason to get the test. You do not need a dementia scare to earn it.


Reversibility depends on the type and cause, not on whether the change happened near menopause. Conductive loss from wax, fluid, or some middle-ear problems can often improve when the blockage or condition is treated. Gradual sensorineural loss from inner-ear or nerve damage is usually permanent. Sudden sensorineural loss is the time-critical exception.

The honest answer nobody wants to give: there is no proven treatment that reverses gradual age-related sensorineural hearing loss. Not HRT. Not a supplement. Not a food plan.

What we do have is real:

  • Treatment of reversible causes such as wax or selected middle-ear problems
  • Prevention of additional noise injury with safer exposure and hearing protection
  • Prescriber-led medication review when the timing raises concern
  • Amplification and assistive technology when testing shows they fit
  • Communication strategies that reduce the daily work even when thresholds do not change

A hearing aid does not return the ear to normal, and not every person with difficulty needs one. It can make speech and environmental sound more usable when properly matched to the loss.

For sudden sensorineural hearing loss, partial or full recovery is possible, but the outcome varies and treatment is time-sensitive. That is the entire reason the red box sits before the explanation.

Some causes may improve. Most gradual sensorineural loss will not reverse. All of it is worth identifying, because “permanent” and “nothing can help” are not the same sentence.


What hearing test should I get, and how do I read it?

Ask for a diagnostic audiologic evaluation, not only a quick screening. The exact battery should match your symptoms, but it commonly includes an ear-canal check, pure-tone and speech testing, and sometimes tympanometry. When speech in noise is the complaint, ask whether speech-in-noise testing can be added. (American Academy of Audiology adult assessment guidance)

Here is what each part does.

Otoscopy — someone looks into the ear canal and at the eardrum. It can find wax and visible outer- or middle-ear clues. It cannot rule out sudden sensorineural hearing loss by itself.

Tympanometry — measures how the eardrum and middle ear respond to a pressure change. It can help identify fluid or middle-ear pressure problems. It is not needed in every case, so ask what your symptoms call for.

Pure-tone audiometry — the beeps. It measures the softest sound you detect at different pitches in each ear and produces the audiogram.

Speech testing — measures how well you detect or repeat speech under controlled conditions.

Speech-in-noise testing — directly tests the complaint many women bring in: hearing a voice when competing sound is present. It may not be part of every routine battery, so name the problem and ask whether the test is available.

Reading the audiogram, in plain terms: the horizontal axis is frequency, from lower pitch on the left to higher pitch on the right. The vertical axis is the hearing level needed for detection; marks farther down generally mean more sound was required. Right- and left-ear results are plotted separately.

A downward slope toward the higher frequencies is common in age- or noise-related loss, but the shape does not diagnose the cause by itself. The two things worth asking about are:

  1. Do the ears differ enough to need medical follow-up?
  2. Do the speech results match the pure-tone thresholds and my real complaint?

Ask for a copy of the audiogram and all speech scores. It gives future testing a documented comparison point instead of forcing everyone to rely on memory.

Ready to get the baseline?

A diagnostic hearing test commits you to nothing. No device. No prescription. No agreement to buy from the clinic. Use this booking sentence: “I have gradual difficulty understanding speech in background noise. I want a diagnostic audiologic evaluation, and I want to ask whether speech-in-noise testing is available.” If the change was sudden, one-sided, painful, draining, or paired with vertigo or neurological symptoms, do not use the routine script. Seek medical assessment first.

There is no honest single U.S. cash price for a hearing test. Your bill depends on the tests performed, the facility, network status, referral rules, and whether the visit is diagnostic or for hearing-aid fitting. Medicare's diagnostic coverage is specific; commercial and Medicare Advantage benefits must be checked plan by plan.

This trips up almost everyone, so let's be blunt about it. A diagnostic evaluation and a hearing-aid fitting may happen in the same clinic, but they are not the same covered service.

Do I need hearing aids, and can I buy them over the counter?

You may not need hearing aids, and you should not choose a device category before you know the pattern. U.S. adults 18 and older with perceived mild-to-moderate loss can buy FDA-regulated OTC hearing aids without an exam or prescription. Severe, sudden, fluctuating, painful, draining, markedly one-sided, or vertigo-associated symptoms need professional assessment.

The OTC category took effect on October 17, 2022. OTC devices are air-conduction hearing aids for adults who perceive mild-to-moderate loss; they can be purchased in stores or online without an ENT visit, prescription, or audiologist fitting. In September 2024, the FDA authorized the first OTC hearing-aid software device: Apple's Hearing Aid Feature for AirPods Pro 2. The authorization was supported by a 118-person study in which self-fitting produced similar perceived benefit to professional fitting of the same device, with comparable in-ear amplification and speech-in-noise performance. Match the professional to the pattern. Gradual difficulty in both ears usually starts with a diagnostic audiology appointment. Sudden, one-sided, painful, draining, or vertigo-associated change needs a physician or ENT first — same day when sudden. Menopause and HRT questions belong with the prescriber and should not delay the ear evaluation.

What should I do about a hearing change this week?

Three actions, in order, none of which require proving menopause caused the change. Book the appropriate hearing evaluation. Build a dated medication-and-symptom timeline. Check whether osteoporosis screening has been addressed according to your age and fracture risk. The point is evidence, not another round of guessing.

Day 1 — Book the right test. If the change was sudden, seek medical care today. If it was gradual, ask for a diagnostic audiologic evaluation and name the real complaint: understanding speech in background noise. Ask whether speech-in-noise testing is available.

Day 1 — Start the two-week log. Use the six listening situations above. Rate the same situations twice so you can tell a persistent pattern from one bad room or one exhausted day.

Day 2 — Build your medication timeline. Include every prescription, over-the-counter medicine, and supplement: product, dose, start date, and any recent change. Include HRT by exact product, route, dose, and start date. Do not stop anything to “test” a theory.

Day 3 — Write down your ear history. Loud work, power tools, firearms, concerts, headphones, infections, injuries, family hearing history, tinnitus, fullness, drainage, pain, and vertigo. Note which ear and whether the beginning was sudden or gradual.

Day 4 — Gather the health facts you already have. Most recent blood pressure and glucose or A1c, if measured. Prior audiograms. Whether osteoporosis screening has been discussed and any fracture history. You are building context, not diagnosing yourself.

At the hearing appointment — ask these five:

  1. What do my thresholds look like in each ear across the full frequencies tested?
  2. Can we test speech in noise because that is where I struggle?
  3. Are my two ears different enough to need medical follow-up?
  4. Is there anything in my dated medication timeline that belongs with the prescriber?
  5. Can I have a copy of the audiogram and all speech scores?

If you use HRT, add these two for the prescriber — in a separate conversation:

  1. Does my current treatment still make sense for the menopause symptom or indication it was prescribed to treat?
  2. Based on my age and fracture-risk history, should osteoporosis screening be addressed?

Notice what is not on either list: “Should I stop my HRT because of my ears?” That is not a safe experiment. Do not let it become one.

The follow-up questions all come back to three distinctions: sudden versus gradual, one ear versus both, and hearing care versus hormone care. Menopause can share the timeline without proving the cause. The answers below close the common gaps without turning a research association into a treatment instruction.

Can perimenopause cause hearing loss before my periods stop? Hearing can change during perimenopause, but perimenopause has not been established as a direct cause. The Korean trajectory study found two group-level patterns around the final menstrual period; it cannot tell one woman that her symptom is hormonal. Get the change assessed regardless of cycle stage.

Can menopause cause hearing loss in one ear? Menopause has not been established as a cause of one-sided hearing loss. A clear new difference between ears — especially a sudden one — deserves medical evaluation rather than a hormone explanation.

Is hearing loss from menopause permanent? Permanence depends on the type. Wax, fluid, and some middle-ear problems can improve; gradual sensorineural damage usually does not reverse. Sudden sensorineural loss can recover partly or fully in some people, but it needs urgent assessment because treatment is time-sensitive.

Will HRT reverse my hearing loss? No human evidence establishes HRT as a treatment that reverses hearing loss, and no FDA-approved menopausal hormone therapy is indicated for hearing. HRT decisions should be based on an appropriate menopause indication and clinician assessment, not an ear symptom.

Should I stop my HRT because of what I read here? No. Do not change a prescribed medication because of a webpage. Symptoms being treated by HRT may return after stopping, and stopping offers no proven hearing benefit; take the concern to the prescriber and evaluate the ears separately.

Is a patch safer than tablets for my hearing? No route has been proven safer for hearing. In the Curhan study, transdermal associations were described as similar to oral associations, although patch use was less common. That is not evidence to switch routes for an ear symptom.

Is progesterone worse for hearing than estrogen? The larger prospective dataset did not support a simple “progestogen harms, estrogen protects” story. Do not remove prescribed endometrial protection because of hearing headlines: for a woman with a uterus using systemic estrogen, adequate progestogen is generally recommended. Recommended low-dose vaginal estrogen is a separate category and generally does not require a progestogen.

Why is hearing so much worse in restaurants than at home? Background noise and competing voices remove cues your brain uses in quiet. A person can detect tones or follow one voice at home yet struggle when speech must be separated from noise. Ask whether speech-in-noise testing fits that complaint.

Can low estrogen cause a blocked or full feeling in my ears? Low estrogen has not been established as the cause of ear fullness. Wax, middle-ear pressure or fluid, Ménière's disease, and sudden sensorineural loss can all involve fullness. Persistent, one-sided, fluctuating, or sudden fullness deserves assessment.

At what age does hearing loss usually start in women? There is no single starting age. Change accumulates differently with age, genetics, noise exposure, illness, and medication history. A baseline becomes useful when you notice difficulty — not when you reach a particular birthday.

Does hearing loss cause dementia? Hearing loss is associated with dementia risk and appears in the 2024 Lancet Commission's risk model, but association is not proof of direct causation. ACHIEVE was negative overall; its cognitive benefit appeared in an older, higher-risk subgroup, not the full sample and not a midlife-menopause population.

Is a free online hearing test good enough? An online or app-based test can be a useful screening prompt. It cannot inspect the ear canal, assess the middle ear, reproduce every clinical speech test, or replace urgent medical evaluation for sudden or one-sided change. A concerning result — or persistent real-world difficulty despite a normal result — deserves a diagnostic evaluation.

Do I need an ENT or an audiologist? Gradual, roughly equal difficulty generally starts with an audiologist. Sudden, one-sided, painful, draining, injured, or vertigo-associated change should start with a physician or ENT. Neurological symptoms need emergency care.

Could a medicine be causing this? It is possible for some medicines to affect hearing or balance, but the answer depends on the exact drug, dose, treatment context, and timing. Do not stop it yourself. Bring a dated list to the prescriber or pharmacist and pair that review with hearing testing.

I had a hysterectomy. Does that change the answer? A hysterectomy removes the uterus; it does not always remove the ovaries or create surgical menopause. Record whether one or both ovaries were removed and at what age. The hearing studies on surgical menopause cannot be applied to “hysterectomy” without that distinction.

Does testosterone or another hormone help hearing in women? There is no good evidence supporting testosterone as a hearing treatment, and no testosterone product is FDA-approved specifically for women in the United States or for hearing loss. Testosterone is a Schedule III controlled substance in the United States and requires a prescription; it should never be presented as a casual hearing intervention.

Is tinnitus the same thing as hearing loss? No. Tinnitus is the perception of sound without an external source; hearing loss is reduced ability to detect or understand sound. They often occur together and can share causes, but the evidence and next steps differ. See menopause and tinnitus.

Your hearing may be changing, and the work it takes to follow dinner-table conversation is real. Menopause shares the timeline, but it has not been established as the independent cause of every midlife hearing change. HRT is not the hearing treatment. The useful next move is to identify the pattern and measure it.

Book the right test. Bring the log. Ask about each ear, the higher frequencies, and speech in noise. Keep the results.
