Tia HRT Cost in 2026: What You’ll Actually Pay

Editorial research — not medically reviewed by a clinician. Why this label
How we researched this:we read Tia’s own membership, terms, insurance, menopause, and city price pages, plus third-party reporting. We did not sign up for, book, or cancel a Tia membership ourselves. The HRT Index has no paid or affiliate relationship with Tia — we earn nothing if you book with them. Midi Health, Hers, and Sesame are affiliate partners: if you use those links we may earn a commission at no cost to you, and it never changes our verdict. Our full disclosure.

Tia HRT cost isn’t one price — it’s four. Tia Essential has no membership fee; Tia Signature is $25 a month ($240 a year). But the membership doesn’t include your care. Your visit, any labs, and any medication are billed separately. Tia lists a $225 cash ratefor “a recurring or ongoing concern” like menopause in its cash-pay markets, and if you use insurance you’ll owe your plan’s share instead.

The part almost no one tells you: a July 2026 investigation co-published by LA Public Press and The 19th found that 17 of 29current and former Tia patients interviewed reported surprise bills, some in the thousands of dollars. Tia says those experiences aren’t the norm. So the real question isn’t “what’s the sticker price?” It’s “what will I actually pay — and how do I build a paper trail if the bill doesn’t match what I was told?”

Best for / not for you

Tia may be a good fit if you:

  • Live in a Tia state and have a PPO plan Tia accepts (it takes most major PPOs).
  • Want menopause care connected to your regular doctor — primary care, gynecology, mental health — in person or by video.
  • Would rather use insurance than buy an all-in-one cash hormone plan.
  • Are willing to confirm your coverage in writing before you book.

Tia is probably not your best starting point if you:

  • Have Medicare, Medicaid, or Medi-Cal— Tia can’t treat you, even if you offer to pay cash.
  • Live outside Tia’s six virtual states.
  • Want one flat monthly price that includes your visits, labs, and medication.
  • Need to know your exact medication and its price before you pay for an assessment.
  • Don’t want a one-year commitment if you pick the paid plan.

The right online HRT provider isn’t the same for every woman — it depends on your symptoms, your age and whether you have a uterus, your medication route preference (patch, pill, gel, or vaginal estrogen), your risk history, your insurance or cash-pay situation, and your state. Some situations belong with an in-person clinician first.

Use The HRT Index’s Find My HRT Path tool to match your situation to the right provider — and to flag when online care isn’t the right starting point — before your first consult. About 90 seconds. No sign-up.

How much does Tia cost for HRT?

Tia HRT care has four cost layers: membership, the visit, labs, and medication. Only the membership is the advertised $0 or $25 a month — the rest is billed separately. Tia lists a $225 cash rate for a “recurring or ongoing concern” like menopause in its cash-pay markets; insured patients pay their plan’s copay, coinsurance, or deductible instead. Any labs and any medication are extra.

Think of it like a phone plan versus your actual phone bill. The membership is the plan. The visits, labs, and prescriptions are the usage. You can’t know your total from the plan price alone. The $25 is the headline number, not the bottom line.

Here’s the one thing that makes budgeting possible: your known starting cost— the floor you can count on before labs, medication, or any extra visit:

Math on Tia’s own published prices. A floor, not a full estimate — labs, medication, and follow-up visits are separate.
Your starting choiceKnown cost floorNot included
Tia Essential + one $225 cash visit$225Labs, medication, any extra visits
Tia Signature (annual) + one $225 cash visit$465 ($240 + $225)Labs, medication, any extra visits
Tia Signature (monthly, full year) + one $225 cash visit$525 ($300 + $225)Labs, medication, any extra visits

Is Tia’s $25 membership the price of HRT?

No. Tia Signature’s $25 a month pays for access and support — priority scheduling, walk-in labs, and help with billing and referrals. It does not pay for your clinical visit. Tia Essential costs nothing and uses the same clinicians; you just get less hand-holding. Under both plans, the actual care is billed on top. In Tia’s own terms, your membership “does not cover or pay for any professional services provided by the Tia Providers.”

Here’s how the two plans compare, straight from Tia’s membership page:

Which plan is cheaper for menopause care?

If you mostly need an assessment and you’re comfortable handling insurance questions yourself, Essentialkeeps your fixed cost at zero. If you expect ongoing back-and-forth — refills, referrals, lab reviews — and you’d happily pay $25 to skip the phone-tag, Signature can be worth it. Paid monthly, Signature runs $300 over the year; paid annually, it’s $240— a $60 difference for the same access. One thing Signature does notbuy you is better medicine. Tia says the clinicians and the standard of care are identical on both plans. You’re paying for convenience, not a different doctor.

The catch most people miss: Signature is a one-year commitment

When you sign up for paid Tia Signature, you’re committing to a full year — even if you pay monthly.

Tia’s Terms of Use (updated May 2025) say it directly: “All Memberships are annual commitments regardless of the Membership payment cadence,” and “even though you will be charged monthly, you are committing to the entire length of your one-year Membership.”

(One exception: older “legacy” month-to-month memberships aren’t locked to a year. But a new sign-up today is the annual kind.)

What does a Tia visit cost without insurance?

Tia publishes real cash prices, and a menopause visit falls under its “$225 recurring or ongoing concern” rate in its four cash-pay markets: Los Angeles, San Francisco, New York, and Arizona. A new-patient visit is also $225. That’s the price of the visit only — it does not include labs, medication, or any follow-up.

Source: asktia.com/service-list-los-angeles, verified July 2026. Cash rates apply if you’re uninsured or the service isn’t covered by your plan.
Tia service (cash / self-pay)Price
New patient visit$225
Recurring or ongoing concern (menopause is Tia’s own example)$225
Lab review & care plan$225
Blood draw appointment$75 (plus the lab’s charge for the tests)

A number worth sitting with:a menopause workup can be more than one visit. One possible path is a $225 first visit to talk through symptoms and order labs, then a separate $225 “lab review & care plan” visit to go over results and start treatment — $450 in visitsbefore a single lab test or one month of medication. Tia doesn’t publish a universal lab requirement or a standard menopause follow-up schedule, so treat that as an illustration, not a required path.

What will Tia HRT cost with insurance?

There’s no single insured price. Tia bills your insurance like any doctor’s office, so you’ll owe whatever your plan says — a copay, coinsurance, or your deductible. Tia states plainly that “covered by insurance does not mean paid for by insurance.” A menopause visit is usually billed as diagnostic (problem) care, which means real out-of-pocket cost, not a free preventive visit.

To help you estimate, Tia publishes average out-of-pocket numbers by city. Read them carefully — they are notamounts you add together, and only some come from Tia’s own patients.

Sources: Tia’s four city service lists, verified July 2026. The $15–$75 copay range is an industry average; coinsurance and deductible figures are from Tia’s 2025 patient data. Estimates only, not a personal quote.
Tia clinic cityCash rateTypical copayCoinsurance (after deductible)Paying toward deductible
New York$225$15–$75~$24~$101
Arizona$225$15–$75~$19~$116
Los Angeles$225$15–$75~$34~$196
San Francisco$225$15–$75~$45~$246
Don’t add the columns together.Your plan and your Explanation of Benefits decide which one applies to each service — and different kinds of cost-sharing can kick in at different points in the year. Early on, before your deductible is met, you may owe the higher “deductible” amount (the plan’s negotiated rate). Once it’s met, you usually drop to a copay or coinsurance.

The trap hiding in your “free” annual exam

Tia says an in-network annual exam is typically covered at 100%. But the moment you bring up a symptom like hot flashes or irregular periods, your insurer can reclassify that part of the visit as a “problem visit” and add a charge. Tia even tells you this and suggests keeping menopause questions to a separate visit. Translation: don’t try to fold your HRT conversation into your “free” checkup, or you may turn a $0 visit into a billed one.

Is Tia in-network for your exact plan?

“Tia accepts most PPOs” is not the same as “Tia is in-network for your specific plan, at your location.” Two things to know:

On Medicare or Medicaid? See what fits your situation →

The 3-minute step that gives you a paper trail if the bill is wrong

Before you book anything at Tia (or honestly, any insurance-billed clinic), get the answers in writing. Copy-paste this to Tia’s team through the portal, and ask your insurer the same:

“I’m considering a menopause visit with Tia. Please confirm in writing: (1) Is the exact Tia clinician and billing entity in-network for my specific plan and location? (2) Will this be billed as a diagnostic/problem visit, and what’s my expected out-of-pocket before and after my deductible? (3) Which lab will you use, and will the labs be billed separately? (4) What are the billing codes so I can price this with my insurer first?”

Save every reply, along with the rep’s name and any reference number. Written confirmation can’t guarantee a clean bill, but it’s the difference between “he said, she said” and evidence you can hold up when you dispute a charge. And that changes everything.

Prefer one flat monthly price with your medication included and no insurance guessing?

Hers offers menopause HRT with FDA-approved estradiol and progesterone starting at $79/month (pills) or $134/month(patch) on a 12-month plan — medication included, no insurance to fight. Confirm the charge schedule at checkout, and check that it serves your state.

Want a known monthly price with visits and messaging built in? Sesame’s menopause program is $59/month($49 for Costco members) and includes video visits as needed, unlimited messaging, and basic labs if ordered (in NY, NJ, RI, and ND you pay the lab directly). Medication is separate, and it’s cash-pay — you certify you’re not a Medicare, Medicaid, or TRICARE beneficiary.

Tia vs. the alternatives: which is cheaper and more predictable?

If predictable pricing matters most, flat-fee providers make your cost knowable before you book. Hers starts at $79/month with medication included. Sesame’s menopause program is $59/month with visits and messaging built in. Midi, like Tia, bills insurance but works in all 50 states. Tia’s edge is whole-body, in-person care with insurance — its trade-off is a bill that’s hard to predict in advance.

Read it this way:

  • Lowest, most predictable number with meds included? A flat plan like Hers wins on simplicity.
  • Known monthly price with visits and messaging? Sesame shows you the number before you commit.
  • Insurance-billed, menopause-focused — but nationwide? Midi covers all 50 states.
  • Whole-body care under one roof, in person, with an accepted PPO? That’s Tia’s lane — just verify coverage in writing first.

What does Tia still not publish about HRT costs?

Tia publishes its membership prices and cash visit rates — which is more than most clinics do — but it does not publish several things you’d need for a complete HRT estimate: a guaranteed medication list, medication prices, a standard menopause lab panel, or a standard follow-up schedule. A visit also doesn’t guarantee a prescription. We’d rather name these gaps than paper over them with a guess.

Confirmed vs. what you’ll have to ask about.

Questions to bring to your first visit: Which HRT routes do you prescribe in my state? Is the product you’re recommending FDA-approved or compounded? Which pharmacy fills it? What labs will you order, and what are the codes? How many follow-ups should I expect, and what does each cost? Ask those, and you’ve closed every gap Tia leaves open.

Is Tia worth the cost for HRT?

Tia can be worth it for a woman in a Tia state, with an accepted PPO, who wants menopause care connected to her broader health — and who’s willing to confirm coverage before booking. It’s a weaker choice for anyone who wants a fixed, all-in cash price with medication included, or who’s on a government plan Tia can’t accept. This is an editorial judgment based on the verified facts above, not a medical recommendation.

We review every provider the same way — with our HRT Index Verification Standard, on five pillars, in the same order, with no scores or star ratings.

1Clinical legitimacy

Real practice. Tia runs in-person and virtual clinics with a dedicated menopause assessment; care is delivered by licensed clinicians — nurse practitioners and a certified nurse-midwife who list menopause and perimenopause as areas of care, with physicians available — working with Tia’s health-system partners (Cedars-Sinai, UCSF, Dignity Health). The asterisk: the July 2026 investigation and an earlier permit lapse are real and worth reading before you commit.

2Care quality

The model’s strength is connection — menopause care that talks to your primary care and mental health, not a hormone-only transaction. Signature adds coordination; Essential uses the same clinical standard. We won’t quote Tia’s testimonials as proof of medical results, and the independent review pool is small and mixed — praise for the in-person experience, real frustration over billing.

3Medication fit

Under-documented publicly. Tia doesn’t publish a formulary, medication prices, or whether products are FDA-approved or compounded, and a visit doesn’t guarantee a prescription. Ask which routes they prescribe in your state, and whether your exact product is FDA-approved or compounded, before you pay.

4Price transparency

Genuinely better than most clinics on the visit — Tia publishes cash rates by city, which is rare. But it’s incomplete for HRT, because medication, labs, follow-up frequency, and the real cost of a mid-year cancellation aren’t assembled anywhere into one number.

5Access

Strong where Tia has clinics or licenses and your insurance is accepted. Weak outside its six virtual states, and closed entirely to Medicare, Medicaid, and Medi-Cal members.

Our verdict:

Tia is a stronger fit as an insurance-friendly, integrated women’s-health home than as a predictable HRT subscription. If you value coordinated care and you’ll do the coverage homework, the separate billing can be worth it. If your first priority is one fixed price you never have to fight, compare a flat-fee provider before you book.

What we verified

We built this page by reading Tia’s own membership, terms, insurance, menopause, and four city price pages, then separating what’s confirmed from what isn’t. Commercial facts are dated to July 2026. Medical and regulatory points cite primary sources. Fit judgments are labeled as our editorial conclusions, not facts.

A few things we could notconfirm, and that you should verify at booking: Tia’s exact hormone list and which routes it prescribes in your state; whether it ever uses compounded products; medication prices; its standard menopause lab panel; and its standard follow-up schedule. We’d rather flag these than guess — because on a health-cost page, a confident wrong number is worse than an honest unknown.

Tia HRT cost FAQ

How much is Tia HRT per month?
There’s no single monthly total. Membership is $0 (Essential) or $25/month (Signature), but visits, labs, and medication are separate. A cash visit for a recurring or ongoing concern like menopause is $225 in Tia’s cash-pay markets; with insurance you owe your plan’s share.
Is Tia’s $25 fee an HRT subscription?
No. It’s a membership for access and support. Your clinical care is billed on top, to you or your insurance.
Is a membership required to book menopause care?
No. You can book with no membership fee under Tia Essential.
How much is a Tia menopause visit without insurance?
$225 in Los Angeles, San Francisco, New York, and Arizona — Tia’s cash rate for a recurring or ongoing concern. That’s the visit only; labs and medication are extra.
Does Tia take insurance for HRT?
Yes — Tia bills most major PPOs. But “covered” doesn’t mean “free”; you’ll owe a copay, coinsurance, or deductible. Tia is out-of-network for HMOs.
Does Tia accept Medicare or Medicaid?
No. Tia can’t treat Medicare, Medicaid, or Medi-Cal members, even on a cash basis.
Are Tia labs included?
No. The lab (Quest, or Quest/Sonora in Arizona) bills you separately. Expect two bills.
Is HRT medication included?
No. You fill it at a pharmacy, billed separately. Tia doesn’t publish a medication price; generics are often inexpensive, but your exact cost depends on the product and pharmacy.
Does Tia prescribe HRT?
Tia offers menopause assessments and says treatment “may include” hormone therapy, but a visit doesn’t guarantee a prescription or a specific route. Ask what they prescribe in your state.
Does Tia prescribe compounded hormones?
Tia’s public pages don’t confirm an FDA-approved-only formulary or whether compounded HRT is ever used. Ask whether your exact prescription is FDA-approved or compounded — the two are not equivalent.
Can I use Tia without insurance?
Yes in California, New York, and Arizona (cash rates). Its other virtual states — Massachusetts, Connecticut, New Jersey — run on in-network insurance; confirm at booking.
What states offer Tia virtual menopause care?
California, New York, Arizona, Massachusetts, Connecticut, and New Jersey, with in-person clinics in Los Angeles, San Francisco, New York, and Scottsdale.
Can I use an HSA or FSA?
For visits and services, generally yes (submit itemized receipts). For the membership fee, generally no. Check with your plan administrator.
How do I cancel a Tia membership?
Through the member portal’s cancel flow. It’s free within 48 hours of signing up. After that, a new paid membership is a one-year commitment: monthly-paid members are charged through the term, cancellation stops the next annual renewal, and it’s non-refundable except for Tia-approved documented circumstances.
Is Tia an annual commitment?
Yes, for new paid memberships — even if you pay monthly.
Is Tia FDA-approved?
A clinic isn’t “FDA-approved” — FDA approval applies to specific drugs. Ask whether your exact prescription is FDA-approved or compounded.
Is this the same as Tia Health in Canada?
No. This is Tia (asktia.com), the U.S. women’s health clinic. Not Tia Health in Canada, and not “TIA,” the medical term for a mini-stroke.
