Does Ambetter Cover HRT for Menopause? What the 2026 Drug Lists Show

By The HRT Index Editorial Team · Editorial research — not medically reviewed · Last updated November 2, 2026 · Last verified November 2, 2026

Does Ambetter cover HRT for menopause? Usually yes, for FDA-approved hormones. In all five 2026 Ambetter drug lists we read, generic estradiol pills, patches and vaginal cream and generic progesterone capsules sit on low-cost Tier 1B. Some brands need prior approval, and your state's list, your plan and your deductible decide what you pay.

The HRT Index is the independent decision resource for online menopause and HRT care — comparing telehealth providers on clinical legitimacy, care quality, medication fit, price transparency, and access, with every claim verified and dated, so women can choose the path that fits their situation before their first consult.

What we actually verified · Last verified November 2026

  • What we read: the 2026 Ambetter formularies (drug lists) for Pennsylvania, Illinois, Arkansas, Nevada and Kentucky, downloaded from ambetterhealth.com on November 2, 2026. Each says it is effective January 1, 2026, and each page footer reads November 2026. We also read Ambetter's pharmacy program page, the Ambetter from Coordinated Care (Washington) pharmacy FAQ, a 2026 Ambetter Evidence of Coverage for Nebraska, a 2025 Ambetter pharmacy tips sheet for Mississippi, the Ambetter from Health Net (California) drug list page, HealthCare.gov's pages on essential health benefits and appeals, and medical guidance from The Menopause Society and the FDA. Every source is listed at the bottom with the date we read it.
  • What we did not do: we did not read the drug list for every Ambetter state, log in to a member account, call Ambetter or check what any person would pay. We don't quote copays or prices, because they depend on your plan's deductible and cost-sharing, which the drug lists don't show.
  • Commercial interest: The HRT Index may earn a commission from some providers featured through The HRT Index's Find My HRT Path tool. Ambetter didn't pay for or review this page, and we have no relationship with Ambetter. See our affiliate disclosure.
  • Verification Standard: we judge online menopause care on five pillars, in this order: clinical legitimacy, care quality, medication fit, price transparency and access. Our methodology explains how.

Does Ambetter cover HRT for menopause? The quick answer by situation

Ambetter plans generally cover the common FDA-approved menopause hormones as prescription drugs, and the cheapest route is usually a generic. Brand-name products tend to sit on a higher tier, and a few need your prescriber to get approval first. The table below sums up what the 2026 drug lists we read show. It's a guide for your call to Ambetter and your talk with your clinician, not a promise of coverage.

Your situation What the 2026 Ambetter drug lists we read show What to do next
You want estrogen for hot flashes or night sweats Generic estradiol tablets and generic estradiol patches (weekly and twice-weekly) are Tier 1B in all five states. Twice-weekly patches have a quantity limit. Ask your prescriber whether a generic pill or patch fits you.
You have a uterus and need progesterone too Generic progesterone capsules (the generic of Prometrium) are Tier 1B in all five. Medroxyprogesterone acetate tablets are Tier 1A or 1B by strength. Ask which progestogen protects your uterine lining.
You were prescribed Bijuva, Duavee or Femring All three are Tier 3. In Illinois, Arkansas, Nevada and Kentucky they also need prior authorization. In Pennsylvania they don't. Check your state's list, then ask about a generic alternative.
You only have vaginal dryness or painful sex Generic estradiol vaginal cream and vaginal tablets are Tier 1B in all five. Premarin vaginal cream is Tier 2 in four states and Tier 0 in Illinois. Ask about low-dose vaginal estrogen.
You were told about Veozah or Lynkuet (nonhormonal) Neither appeared in any of the five drug lists we read. Ask your prescriber about a formulary exception or a listed alternative.
You were offered compounded hormones or pellets Ambetter's drug lists cover FDA-approved drugs. Compounded drugs are handled under separate, plan-specific rules. Read your plan's Evidence of Coverage and call Member Services first.
Your plan is Ambetter from Health Net (California) California uses its own Ambetter from Health Net drug list, which we didn't review. Use that drug list or call Member Services.

Bottom line: on the Ambetter drug lists we read, the generic estradiol and progesterone most women start with are covered on a low tier. What you pay at the pharmacy still depends on your plan's deductible and copays, which you'll find in your own plan documents.

The right online menopause care isn't the same for every woman — it depends on your symptoms, what you've already tried, your age, your state and your health history. Some situations belong with an in-person clinician first. Use The HRT Index's Find My HRT Path tool to explore a featured care option for your situation — and to flag when online care isn't the right starting point — before your first consult.

Find My HRT Path is free, takes about 60 seconds and doesn't ask for your email. It asks five quick questions about your symptoms, what you've already tried, your age, your state and your health history. It isn't an insurance check, an eligibility test, a diagnosis or a prescription decision, and it can't tell you what Ambetter will pay.

What is Ambetter, and why does your state matter?

Ambetter Health is a brand of individual and family health plans from Centene Corporation, including many plans sold on the Health Insurance Marketplace (HealthCare.gov or your state's exchange). Each state's Ambetter plan publishes its own drug list, so the coverage details can differ from state to state.

Ambetter says it has been "offering health insurance plans across the U.S., including many options on the Health Insurance Marketplace" since 2014, and that it "serves more than 3.5 million members." Its pharmacy program page lists drug lists for 29 states, from Alabama to Washington. Some state plans carry a local name, such as Ambetter from Coordinated Care in Washington or Ambetter from Health Net in California.

Because Ambetter plans are Marketplace plans, they must cover the 10 essential health benefits, and HealthCare.gov lists prescription drugs among them. That's why the useful question isn't "does Ambetter cover hormones at all?" but "which hormone, at which tier, with which rules, on my state's list?"

This page is about Ambetter Marketplace plans. If you have Medicaid, Medicare or a job-based plan, see does Medicaid cover HRT for menopause?, does Medicare cover HRT for menopause? or our general guide, does insurance cover HRT for menopause?. Ambetter 2026 drug list: menopause hormones by state

We searched the 2026 Ambetter formularies for Pennsylvania, Illinois, Arkansas, Nevada and Kentucky for every menopause hormone product we could think of. Generic estradiol and progesterone were on the same low tier in all five; the differences showed up in a few brand-name products, mostly in whether prior authorization is required.

How to read the table: the tier sets your cost-sharing level (lower is usually cheaper), "prior auth" means your prescriber must get Ambetter's approval first, and "quantity limit" means Ambetter covers only a set amount. Brand names are in capitals, as on the drug lists. "Not listed" means we didn't find it in that state's PDF; Ambetter says the list "is not meant to be a complete list," so call Member Services before you assume a drug isn't covered.

Source: the 2026 Ambetter formularies for each state, read November 2, 2026 (links in Sources). Arkansas, Nevada and Kentucky showed the same tier and requirement for every row above, so we combined them.

What jumps out:

  • Generics are the easy path. Generic estradiol pills, patches and vaginal cream, plus generic progesterone capsules, are on Tier 1B everywhere we looked. Ambetter's drug list says generics "should be considered the first line of treatment."
  • Brand-name combinations cost more and may need approval. Bijuva and Duavee are Tier 3 everywhere, with prior authorization in four of the five states.
  • The same product can have different rules by state. Femring needs prior authorization in four states but not in Pennsylvania. Premarin vaginal cream is Tier 0 in Illinois and Tier 2 elsewhere.
  • Some brands we searched for weren't on any list, including Prometrium (the generic progesterone is listed), Vagifem and Yuvafem (generic vaginal tablets are listed), Imvexxy, Divigel, EstroGel, Activella and Intrarosa. That doesn't always mean "never covered." It means you'd likely use the listed generic or ask for an exception.

If your state isn't one of these five, you can run the same search yourself: see how to check your Ambetter HRT coverage below. For a product-by-product look at coverage across insurers, see what HRT is covered by insurance?, does insurance cover progesterone for menopause? and does insurance cover the estradiol patch?.

What Ambetter's tiers and codes mean for your cost

Ambetter's drug lists sort covered drugs into tiers from 0 to 4. Lower tiers have lower cost-sharing, but the drug list doesn't show dollar amounts; your plan's Schedule of Benefits or Summary of Benefits does.

Here's how Ambetter's 2026 Pennsylvania formulary defines each tier and code (the other four states use the same key):

Tier or code Ambetter's definition (quoted) What it means for HRT
Tier 0 "No copayment for those drugs that are used for prevention and are mandated by the Affordable Care Act." Mostly preventive drugs like select birth control. Of the menopause products in our table, only Premarin vaginal cream in Illinois was on Tier 0.
Tier 1A "Lowest copayment for select drugs that offer the greatest value compared to other drugs used to treat similar conditions." Medroxyprogesterone acetate 10 mg.
Tier 1B "Low copayment for those drugs that offer great value compared to other drugs used to treat similar conditions." Generic estradiol pills, patches and vaginal cream; generic progesterone capsules.
Tier 2 "Medium copayment covers brand name drugs that are generally more affordable, or may be preferred compared to other drugs to treat the same conditions." Premarin tablets, Prempro, Premarin vaginal cream in most states.
Tier 3 "High copayment covers higher cost brand name and non-preferred generic drugs." Bijuva, Duavee, Femring, Evamist, Climara Pro.
Tier 4 "Highest copayment is for 'specialty' drugs used to treat complex, chronic conditions." No menopause hormone we checked was on Tier 4.
PA "Your doctor must ask for approval from Ambetter before some drugs will be covered." Prior authorization.
QL "Some drugs are only covered for a certain amount." Quantity limit, such as a daily maximum of cream.
ST Step therapy: "In some cases, you must first try certain drugs before Ambetter covers another drug for your medical condition." None of the menopause rows we checked showed this code.

Three things the tier doesn't tell you:

  1. Your deductible. Depending on your plan, you may pay the full negotiated price until you meet your deductible. Check your plan's Schedule of Benefits.
  2. Whether your plan design excludes something anyway. The drug list itself warns: "Specific prescription benefit plan designs may not cover certain products or categories, regardless of their appearance in this document."
  3. Mid-year changes. The list "is periodically reviewed and updated and may be subject to change." Ambetter from Coordinated Care in Washington says it reviews its formulary quarterly and that "changes that negatively impact the cost of your prescription will only be implemented once a year at the start of a new benefit year."

A practical tip: a generic on Tier 1B may cost less through your plan than a brand on Tier 3, but if you haven't met your deductible, compare with a cash price too. See estradiol cost without insurance, cheapest progesterone without insurance and Cost Plus Drugs for HRT.

Does Ambetter need prior authorization for HRT?

For the generic estradiol and progesterone products on the 2026 lists we read, no prior authorization code was shown. Some brand-name products, including Bijuva, Duavee and Femring in four of the five states and Osphena in all five, need your prescriber to get approval from Ambetter first.

Prior authorization is your prescriber's job, not yours, but you can speed it up. Ambetter's formulary says that for non-formulary drug requests, it will respond "within 24 hours of receipt of all necessary information for urgent requests, and within 72 hours for non-urgent requests, unless state law requires faster response." If it says no, the notice "will contain a clear explanation of the specific reasons."

What usually helps a request, based on how Ambetter's documents describe the process:

  • A clear reason the listed option doesn't fit. A 2025 Ambetter pharmacy tips sheet for Mississippi says non-formulary medications "require documentation of a trial and failure of at least two formulary alternatives as part of the PA review." Rules vary by state, but the idea is the same: show why a listed drug won't work for you.
  • The right form, sent by your prescriber. Ambetter's state sites list prior authorization contacts for prescribers.
  • Follow-up. If the request was incomplete, Ambetter says its explanation "will identify the missing material information."

For a step-by-step on menopause prior authorizations, see HRT prior authorization for menopause, progesterone prior authorization and estradiol patch prior authorization.

Does Ambetter cover vaginal estrogen?

Yes, on the five 2026 lists we read. Generic estradiol vaginal cream (with a daily quantity limit) and generic estradiol vaginal tablets are Tier 1B, and Premarin vaginal cream is Tier 2 in four states and Tier 0 in Illinois. The Estring ring was listed only in Pennsylvania, at Tier 3.

Vaginal estrogen matters because vaginal dryness, irritation and painful sex (part of the genitourinary syndrome of menopause, or GSM) are common and often don't improve on their own. The Menopause Society's 2022 position statement says that for these symptoms, when over-the-counter therapies don't help and a woman doesn't need systemic hormone therapy, "low-dose vaginal estrogen therapy or other therapies (eg, vaginal dehydroepiandrosterone or oral ospemifene) are recommended." Ospemifene is the drug in Osphena, which is Tier 3 with prior authorization on all five lists.

Be clear with your prescriber about which ring you mean. Estring is a low-dose ring for vaginal symptoms. Femring is a higher-dose ring that treats hot flashes throughout the body, so if you have a uterus it is generally paired with a progestogen. On Ambetter's lists, Femring needs prior authorization in four of the five states.

Does Ambetter cover Veozah or other nonhormonal options?

We didn't find Veozah (fezolinetant) or Lynkuet (elinzanetant) on any of the five 2026 Ambetter drug lists we read. That doesn't mean they're never covered. Your prescriber can ask for an exception, and Ambetter's Evidence of Coverage explains that process.

The 2026 Nebraska Evidence of Coverage says: "Under the Affordable Care Act, you have the right to request coverage of prescription drugs that are not listed on the plan formulary (otherwise known as 'non-formulary drugs')." It describes a standard exception decided "within 72 hours," an expedited exception decided "within 24 hours" when your health is seriously at risk, and a review by "an independent review organization" if Ambetter denies the exception. Ambetter from Coordinated Care in Washington puts it this way: "According to the Affordable Care Act (ACA), we cannot exclude any drug. Prescription drugs not available on the 2026 Formulary/Prescription Drug List (PDF) can be reviewed through an exception process."

If you can't or prefer not to take hormones, ask your clinician which nonhormonal options are on your list and which would need an exception. See does insurance cover Veozah?, does insurance cover Lynkuet?, Veozah prior authorization and our overview of nonhormonal options.

Does Ambetter cover compounded bioidentical hormones or pellets?

Ambetter's drug lists are lists of FDA-approved drugs, so compounded hormones aren't on them. Whether a compounded prescription is paid at all depends on your state's plan documents, and the ones we read set limits. Hormone pellets weren't addressed in the pharmacy documents we read, so check with Ambetter before you agree to them.

What the documents say:

  • The drug list: "The Ambetter Formulary, or Prescription Drug List, is a guide to available brand and generic drugs that are approved by the Food and Drug Administration (FDA)."
  • A 2026 Evidence of Coverage (Nebraska) lists among its pharmacy exclusions: "Compound drugs, unless there is at least one ingredient that is an FDA approved drug." Read your own plan's wording; it may differ.
  • A 2025 Ambetter pharmacy tips sheet (Mississippi) lists "Compounded drugs" as an example of medications "excluded from coverage under Ambetter benefits."

So the answer can differ by state, and even "covered" may mean prior authorization or a higher tier. Ask Ambetter for the written rule that applies to your plan before you pay for a compounded product.

On the medical side, the FDA says "compounded drugs are not FDA-approved" and "FDA recommends that women use hormone therapies that are FDA-approved." The good news if you want "bioidentical" hormones: estradiol and micronized progesterone are bioidentical, and their generics are on Ambetter's Tier 1B. See FDA-approved bioidentical hormones, does insurance cover compounded hormones?, does insurance cover hormone pellets? and bioidentical hormone therapy cost.

Does Ambetter cover the menopause visit and labs?

The drug list covers only the medication. Your visit and any tests fall under your plan's medical benefits, where your deductible, copays and network rules apply. The Menopause Society says hormone blood tests are rarely needed for hormone therapy, so ask before paying for extra testing.

Things to check in your plan documents or with Member Services:

  • Network. Is your gynecologist, primary care clinician or menopause specialist in your Ambetter network? The 2026 Nebraska Evidence of Coverage says, outside of balance-billing protections, "we do not normally cover services received from non-network providers." Your own plan's document has its rule.
  • Virtual care. The 2026 Nebraska Evidence of Coverage describes an "Ambetter-designated telehealth provider" for "Virtual Primary Care and Virtual 24/7 Care benefits." Ask whether your plan includes virtual primary care and whether that clinician can manage menopause symptoms.
  • Preventive visits. HealthCare.gov's list of covered preventive services for women includes osteoporosis screening "for women over 65 years or women 64 years and younger who have gone through menopause." Treatment visits for menopause symptoms aren't automatically free preventive care.
  • Hormone tests. The Menopause Society's 2022 statement says salivary and urine hormone testing to set dosing is unreliable and not recommended, and that serum testing is rarely needed.

If you plan to use an online menopause service, ask whether it's in network with your Ambetter plan. Ambetter's pharmacy program page says it covers prescription medications "when ordered by an Ambetter Health provider," so also ask Member Services whether a prescription written by an out-of-network clinician would be covered at your pharmacy. See online HRT providers that accept insurance and online HRT self-pay cost.

How to check your Ambetter HRT coverage in five steps

You can check your exact Ambetter coverage with your member ID card and the name of the medication. The drug list tells you the tier and rules; your plan's Schedule of Benefits tells you the cost for that tier.

  1. Find your state's 2026 drug list. Go to Ambetter's pharmacy program page, pick your state and open the formulary PDF. Make sure it's for the current year.
  2. Search the PDF for the generic name. Use your browser's find tool for "estradiol," "progesterone" or "medroxyprogesterone." Brands are in capitals; generics are lowercase. Look at the form too (TABS, PTWK, PTTW, GEL, vaginal CREA).
  3. Write down the tier and any codes. Note the tier number and whether it shows PA, QL or ST.
  4. Look up your cost for that tier. Find your plan's Schedule of Benefits or Summary of Benefits (in your member account) and check whether prescription drugs are subject to the deductible.
  5. Call Member Services to confirm. The number is on your ID card. Ambetter's own drug list says it isn't complete, so confirm anything not listed.

Questions to ask Member Services, word for word:

  • "Is [medication, strength and form] on my plan's 2026 formulary, and what tier is it?"
  • "Does it need prior authorization, step therapy or a quantity limit?"
  • "Do I have to meet my deductible before my prescription copay applies?"
  • "If it's not on the formulary, how does my prescriber request an exception?"
  • "Is [clinician or online service] in my network?"

Ready to compare care options with these answers in hand? This is a good time to see what kind of care fits you.

What to do if Ambetter denies your HRT

If Ambetter won't cover a menopause medication, you have options: switch to a listed alternative, ask for a formulary exception, appeal the decision, and then ask for an outside review. Many denials are solved at step one by moving to a covered generic.

Step Who does it Timeline from the source
1. Ask about a listed alternative You and your prescriber Often the fastest fix, such as generic estradiol instead of a brand
2. Prior authorization or exception request Your prescriber (you can also request an exception in some plans) Ambetter: 72 hours standard, 24 hours expedited (2026 Nebraska Evidence of Coverage)
3. Internal appeal You or your representative HealthCare.gov: file "within 180 days (6 months)" of the denial
4. External review An independent third party HealthCare.gov: file "within 4 months" of the final denial

HealthCare.gov explains the stakes plainly: with external review, "the insurance company no longer gets the final say over whether to pay a claim," and "your insurer is required by law to accept the external reviewer's decision." Your denial letter and your Evidence of Coverage list the exact steps and deadlines for your plan.

Tools that help: what to do if insurance denies HRT and our HRT insurance appeal letter template.

Coverage isn't the same as fit: is HRT right for you?

Ambetter covering a hormone doesn't mean it's the right choice for you. That's a decision for you and a clinician who knows your health history, including your uterus status and any history of breast cancer, blood clots, stroke, heart disease or liver disease.

What the medical guidance says, in short:

  • It works. The Menopause Society's 2022 position statement says "hormone therapy remains the most effective treatment for vasomotor symptoms (VMS) and the genitourinary syndrome of menopause and has been shown to prevent bone loss and fracture."
  • If you have a uterus, estrogen needs a progestogen. The same statement says women "with an intact uterus using systemic ET should receive adequate progestogen." That's why progesterone or medroxyprogesterone appears next to estradiol on many prescriptions. See do you need progesterone if you have a uterus?.
  • Some women shouldn't use it. History of breast cancer, blood clots, stroke, heart attack, liver disease or unexplained vaginal bleeding are common reasons hormone therapy may not be used. See who should not take HRT and HRT contraindications.

Don't start, stop or switch hormones because of a coverage change without talking with your prescriber. If your plan moves a drug to a higher tier, your prescriber can often switch you to a covered equivalent safely.

Can you use Ambetter with online menopause care?

It depends on the network. Ambetter's pharmacy program page says it covers prescriptions "when ordered by an Ambetter Health provider," so an online service that isn't in your Ambetter network may leave you paying for the visit, and possibly the medication, yourself. Ask Member Services before your first consult.

Here's how our five pillars apply when you have Ambetter:

  • Clinical legitimacy: a licensed clinician in your state should take a full history before prescribing.
  • Care quality: the service should explain FDA-approved options first and tell you why if it suggests a compounded product, which Ambetter may not cover.
  • Medication fit: look for generic estradiol, generic progesterone and generic vaginal estrogen, which are Tier 1B on the Ambetter lists we read.
  • Price transparency: you should know the visit fee and whether prescriptions go to an Ambetter network pharmacy or a cash-pay pharmacy.
  • Access: check that the service operates in your state and, if you want the visit covered, that it's in your Ambetter network.

Frequently asked questions

Does Ambetter cover HRT for menopause?

Generally yes, for FDA-approved hormones. On the five 2026 Ambetter drug lists we read (Pennsylvania, Illinois, Arkansas, Nevada and Kentucky), generic estradiol pills, patches and vaginal cream and generic progesterone capsules were Tier 1B. Your cost depends on your plan's deductible and copays.

Does Ambetter cover estradiol?

Yes, on the lists we read. Generic estradiol tablets, weekly and twice-weekly patches, and vaginal cream and tablets were Tier 1B. Generic estradiol gel was listed twice, at Tier 1B and Tier 3. The Evamist spray was Tier 3.

Does Ambetter cover progesterone?

Yes. Generic progesterone capsules (the generic of Prometrium) were Tier 1B on all five lists. The brand Prometrium wasn't listed. Medroxyprogesterone acetate tablets were Tier 1A or 1B depending on strength.

Does Ambetter cover Bijuva?

Bijuva was Tier 3 on all five lists, and it needed prior authorization in Illinois, Arkansas, Nevada and Kentucky but not Pennsylvania. Ask your prescriber whether generic estradiol plus generic progesterone would work for you.

Is HRT free with Ambetter?

Not usually. Tier 0 is for preventive drugs required by the Affordable Care Act, and the menopause hormones we checked were on paid tiers, except Premarin vaginal cream in Illinois, which was Tier 0. Most women pay a copay or coinsurance, and possibly the deductible first.

Does Ambetter cover compounded bioidentical hormones?

Compounded hormones aren't on Ambetter's drug lists, which cover FDA-approved drugs. A 2026 Ambetter Evidence of Coverage we read excludes compound drugs unless at least one ingredient is FDA-approved, and a 2025 tips sheet lists compounded drugs as excluded. Ask Ambetter for your plan's written rule.

Does Ambetter cover Veozah?

Veozah wasn't on any of the five 2026 lists we read. Your prescriber can request a non-formulary exception, which Ambetter's 2026 Evidence of Coverage says it decides within 72 hours (24 hours if expedited).

Can I see any doctor for HRT with Ambetter?

Check your plan's network first. The 2026 Ambetter Evidence of Coverage we read says it does "not normally cover services received from non-network providers," and some Ambetter plans include virtual primary care through a designated telehealth provider.

Can Find My HRT Path tell me what Ambetter covers?

No. Find My HRT Path asks about your symptoms, what you've tried, your age, your state and your health history to suggest a care option. It doesn't check insurance. Ambetter Member Services and your plan documents do that.

The bottom line

Does Ambetter cover HRT for menopause? On the 2026 Ambetter drug lists we read, the answer is yes for the basics: generic estradiol pills, patches and vaginal cream and generic progesterone capsules are Tier 1B in all five states. Brand-name combinations like Bijuva and Duavee are Tier 3 and often need prior authorization, nonhormonal Veozah and Lynkuet weren't listed, and compounded hormones fall under separate plan rules. Check your own state's list, your deductible and your network, and ask your prescriber whether a covered generic fits you.

If you've sorted out coverage but still aren't sure which kind of care to start with, Find My HRT Path can point you to a starting option and flag when in-person care should come first.
